Please submit all of the items listed below as soon as you are approved and scheduled for a rotation in Chattanooga:

UTHSC housing students only:

- Apartment Review: Check-In Form- complete at orientation
- No Smoking/No Damage Form - complete at orientation
- Apartment Checkout Review - complete at rotation checkout

Items to return:
At the end of the rotation, all visiting students must return these items issued at orientation:
- Photo ID badge
- Badge reel
- Meal card
- Erlanger security access cardÌý(if issued)
- Parking pass
Note: Chattanooga-based students are exempt from returning these items.
