Job Description

This position plays a pivotal role in educating individuals and communities about breast, prostate, and colorectal cancer and promoting awareness of screening tests.

  1. Provides education to individuals about breast, prostate, and colorectal cancer, including risk factors, prevention strategies, and the importance of early detection through screening tests.
  2. Distributes educational materials.
  3. Administers breast, colorectal, and prostate cancer screening questionnaires to participants during mobile van outreach events.Ìý

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS:Ìý Under the supervision of the Advanced Practice Registered Nurse (APRN), the Registered Nurse participates in clinical care related to a Health Resources and Services Administration (HRSA) grant. This position assists with the delivery of care at mobile clinic sites, primarily in Dyer, Hardeman, and Haywood counties, including but not limited to, providing information, assisting patients, recording medical histories, assisting with SDoH assessments, and performing preliminary assessments and physical tests. This position drives the Mobile Health Unit, assuring daily operational logs are complete, maintaining medical supplies, inventory, and performing preventive maintenance to keep medical equipment operating.

Responsibilities
  1. Drives the mobile health unit to the site of service, refuels as needed, and empties the gray water tank.
  2. Conducts patient health history assessment, obtains vitals, monitors patients for change in status, communicates findings to APRN, contributes to and implements care plans, and provides patient education.Ìý
  3. Performs finger sticks and draws blood for laboratory specimens according to protocol.
  4. Coordinates patient care, schedules patient appointments, triages patient phone calls, schedules referrals, and administers tests under clinician orders.Ìý

The purpose of the grant is to establish a Mobile Health Unit (MHU) Program that delivers innovative, evidence-based health care services to rural populations in Hardeman, Haywood, and Dyer counties onsite and neighboring counties through telehealth. This position delivers care at mobile clinic sites and elsewhere as needed, including, but not limited to, performing physical examinations, diagnosing illness, taking vitals, prescribing medications, scheduling follow-up appointments, and providing health care training on various public health topics.

JOB SUMMARY/ESSENTIAL JOB FUNCTIONS:Ìý Under the oversight of the Principal Investigator, the Nurse Practitioner participates in all aspects of health care and community and nursing education related to a large Tennessee Department of Health grant. The purpose of the grant is to establish a Mobile Health Unit (MHU) Program that delivers innovative, evidence-based healthcare services to rural populations in Hardeman, Haywood, and Dyer counties onsite and neighboring counties through telehealth. This position delivers care at mobile clinic sites and elsewhere as needed, including, but not limited to, performing physical examinations, diagnosing illness, taking vitals, prescribing medications, scheduling follow-up appointments, and providing healthcare training on various public health topics.

Responsibilities

  1. Performs physical examinations of patients to evaluate health and diagnose illnesses
  2. Orders and interprets laboratory tests.
  3. Determines, plans, and initiates appropriate treatment, and adjusts treatment as necessary.
  4. Prescribe medications.
  5. Documents medical information of patients and maintains electronic medical records.
  6. Educates, instructs, and counsels patients and families on presenting health problems, treatment plans, and home care.
  7. Requests consultation or referrals with other health care providers as needed.
  8. Schedules follow-up appointments.
  9. Encourages and promotes preventive health care and positive health behaviors.Ìý
  10. Drives the mobile clinic to the site of service and maintains the unit for service.
  11. Assists the research team in gathering data for grant reporting.Ìý
  12. Performs other related duties as needed.Ìý

Qualifications

MINIMUM REQUIREMENTS:Ìý

EDUCATION:ÌýMaster’s Degree in Nursing.Ìý(TRANSCRIPT REQUIRED)

EXPERIENCE:ÌýTwo (2) years of clinical work experience. Must have a valid Driver’s License and personal vehicle insurance.

LICENSE/CERTIFICATION:ÌýCurrent license to practice as a Nurse Practitioner in the State of Tennessee. Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP). Must meet all state requirements and obtain valid PrescriptiveÌýAuthorityÌýincluding a DEA certificate.Ìý(COPY OF LICENSE/CERTIFICATION REQUIRED)ÌýÌýÌý
